Medicaid and Badgercare
You are required by the State of Wisconsin to present your Forward Card at time of registration for services. You are also required to pay your co-pay at the time services are provided. Wisconsin Medicaid offers a variety of subprograms. It is your responsibility to know what type of coverage you are eligible for.
- Family Services will cover only services for Family Planning.
- Family Care for long-term care benefits is available in selected counties.
- SeniorCare is a prescription drug assistance program for Wisconsin residents who are 65 years of age or older.
- Well Woman Medicaid is for women 35 to 64 who are in need of treatment for breast or cervical cancer.
Medicare
The Medicare deductible, coinsurance, private room differential and all non-covered services are your responsibility. Medicare generally does not cover preventive care services, private duty nursing, hospital room telephone or television, private hospital rooms, acupuncture, long term or custodial care in nursing homes, most prescription drugs and most drugs that are taken orally (self administered drugs) and given in the hospital.
In addition to original Medicare,; there are several Medicare Advantage Plans; available such as Blue Cross Smart Value, Humana Gold Choice and Network Platinum Plus. It is important to remember that these policies are not supplements or secondary insurances – they are a replacement policy for original Medicare. The claims are submitted directly to the Advantage Plan, not to original Medicare.
Group or Private Insurance
Please present all of your insurance information and identification cards when registering. Any balance remaining after all insurance has paid is due in full from you.
Auto and other Liability
If you received services as a result of an injury due to an automobile accident or other third party liability, we will bill your health insurance carrier. If you have auto or liability insurance, we will send you any billing forms you need, upon request. The only exception would be if Medicare is your primary insurance.
Workers’ Compensation
Workers’ Compensation claims will be filed after verification with your employer.
